Subject: DiscNW Spring League Paperwork
Dear DiscNW Youth Teams,
Attached you will find the required paperwork for the 2010 DiscNW youth spring season.
The first attachment is the DiscNW Coaching Code of Conduct. This is a new form for 2010 which outlines our expectations for coaches in DiscNW events.
The second attachment is the DiscNW Youth League Roster. This is also a new requirement for 2010. This roster can be completed electronically and emailed to your league coordinator, or submitted as a hard copy along with the other two items listed here.
The third attachment is the revised DiscNW Youth Waiver. Note that this is a new waiver for 2010. The waiver can be completed electronically, except for the signatures. Note that the waiver has two parts. The first page is the DiscNW waiver. The second and third pages are the Concussion Information Sheet, newly required by the Lystedt Law.
